A New York Times analysis dissects what went wrong last week in Louisiana:
The governor of Louisiana was "blistering mad." It was the third night after Hurricane Katrina drowned New Orleans, and Gov. Kathleen Babineaux Blanco needed buses to rescue thousands of people from the fetid Superdome and convention center. But only a fraction of the 500 vehicles promised by federal authorities had arrived. Ms. Blanco burst into the state's emergency center in Baton Rouge. "Does anybody in this building know anything about buses?" she recalled crying out.
They were an obvious linchpin for evacuating a city where nearly 100,000 people had no cars. Yet the federal, state and local officials who had failed to round up buses in advance were now in a frantic hunt. It would be two more days before they found enough to empty the shelters.
The official autopsies of the flawed response to the catastrophic storm have already begun in Washington, and may offer lessons for dealing with a terrorist attack or even another hurricane this season. But an initial examination of Katrina's aftermath demonstrates the extent to which the federal government failed to fulfill the pledge it made after the Sept. 11 attacks to face domestic threats as a unified, seamless force.
Instead, the crisis in New Orleans deepened because of a virtual standoff between hesitant federal officials and besieged authorities in Louisiana, interviews with dozens of officials show.
Federal Emergency Management Agency officials expected the state and city to direct their own efforts and ask for help as needed. Leaders in Louisiana and New Orleans, though, were so overwhelmed by the scale of the storm that they were not only unable to manage the crisis, but they were not always exactly sure what they needed. While local officials assumed that Washington would provide rapid and considerable aid, federal officials, weighing legalities and logistics, proceeded at a deliberate pace.
FEMA appears to have underestimated the storm, despite an extraordinary warning from the National Hurricane Center that it could cause "human suffering incredible by modern standards." The agency dispatched only 7 of its 28 urban search and rescue teams to the area before the storm hit and sent no workers at all into New Orleans until after Katrina passed on Monday, Aug. 29.
On Tuesday, a FEMA official who had just flown over the ravaged city by helicopter seemed to have trouble conveying to his bosses the degree of destruction, according to a New Orleans city councilwoman.
"He got on the phone to Washington, and I heard him say, 'You've got to understand how serious this is, and this is not what they're telling me, this is what I saw myself,' " the councilwoman, Cynthia Hedge-Morrell, recalled.
State and federal officials had spent two years working on a disaster plan to prepare for a massive storm, but it was incomplete and had failed to deal with two issues that proved most critical: transporting evacuees and imposing law and order.

The Time magazine analysis of the relief breakdown reports that the White House simply did not respond to Blanco's urgent requests. The day the storm hit, [Blanco] asked President Bush for "everything you've got." But almost nothing arrived, and she couldn't wait any longer. So she called the White House and demanded to speak to the President. George Bush could not be located, two Louisiana officials told Time, so she asked for chief of staff Andrew Card, who was also unavailable. Finally, after being passed to another office or two, she left a message with DHS adviser Frances Frago Townsend. She waited hours but had to make another call herself before she finally got Bush on the line. "Help is on the way," he told her. Full article

Still another analysis of what went wrong, this one in the Washington Post. So much for that big "coordination" role in FEMA's mission statement. There was no coordination: In July 2004, Maestri had participated in an exercise called Hurricane Pam, a simulation of a Category 3 storm drowning New Orleans. Emergency planners had concluded that a real Pam would create a flood of unimaginable proportions, killing tens of thousands of people, wiping out hundreds of thousands of homes, shutting down southeast Louisiana for months. The practice run for a New Orleans apocalypse had been commissioned by the Federal Emergency Management Agency, the federal government's designated disaster shop. But the funding ran out and the doomsday scenario became just another prescient -- but buried -- government report. Now, practice was over. And Pam's lessons had not been learned. As the floodwaters recede and the dead are counted, what went wrong during a terrible week that would render a modern American metropolis of nearly half a million people uninhabitable and set off the largest exodus of people since the Civil War, is starting to become clear. Federal, state and local officials failed to heed forecasts of disaster from hurricane experts. Evacuation plans, never practical, were scrapped entirely for New Orleans's poorest and least able. And once floodwaters rose, as had been long predicted, the rescue teams, medical personnel and emergency power necessary to fight back were nowhere to be found. Compounding the natural catastrophe was a man-made one: the inability of the federal, state and local governments to work together in the face of a disaster long foretold. In many cases, resources that were available were not used, whether Amtrak trains that could have taken evacuees to safety before the storm or the U.S. military's 82nd Airborne division, which spent days on standby waiting for orders that never came. Communications were so impossible the Army Corps of Engineers was unable to inform the rest of the government for crucial hours that levees in New Orleans had been breached. The massive rescue effort that resulted was a fugue of improvisation, by fleets of small boats that set sail off highway underpasses and angry airport directors and daredevil helicopter pilots. Tens of thousands were saved as the city swamped; they were plucked from rooftops and bused, eventually, out of the disaster zone. But it was an infuriating time of challenge when government seemed unable to meet its basic compact with its citizens. After the Sept. 11, 2001, terrorist attacks, an entirely new Department of Homeland Security had been created, charged with doing better the next time, whether the crisis was another terrorist attack or not. Its new plan for safeguarding the nation, unveiled just this year, clearly spelled out the need to take charge in assisting state and local governments sure to be "overwhelmed" by a cataclysmic event. Instead, confusion reigned at every level of officialdom, according to dozens of interviews with participants in Louisiana, Mississippi and Washington. "No one had access. . . . No one had communication. . . . Nobody knew where the people were," recalled Secretary of Health and Human Services Mike Leavitt, whose department did not declare the Gulf Coast a public health emergency until two days after the storm.
